POPULATION HEALTH OPERATIONS CONSULTANT

JOB OVERVIEW
The Population Health Operations Consultant at TriHealth plays a crucial role in improving the delivery of value-based care at TriHealth through program and project management. This position involves managing multiple projects, developing strong relationships with stakeholders, and ensuring effective operational management of health programs. The consultant will work collaboratively to enhance organizational performance and contribute to a positive work environment.

Education
Bachelor's Degree in Related Field Required
Master's Degree in related field Preferred
 

Work Experience

4 - 5 years' Experience in related field 

Job Responsibilities
Essential Function
•    Provide program and project management implementation and execution.
•    Manage multiple projects simultaneously with accountability for ongoing activities and objectives.
•    Serve as an internal resource, consultant, and liaison to patients, families, staff, and community for program-related issues.

•    Direct the ongoing development, implementation, coordination, marketing, and evaluation of program outcomes.
•    Develop and maintain strong working relationships with internal and external partners and stakeholders.

•    Facilitate effective prioritization and communication among teams and community leadership.
•    Direct and coordinate daily operations of the program in collaboration with leadership.

•    Identify areas for improvement in business processes and operational efficiency.
•    Participate in improving organizational performance through recommending improvement activities and collecting data.

•    Work collaboratively with others to achieve goals and promote a positive work environment.
