Job Overview:

This position is responsible to the Manager of Care of Management.  This position is repsonsible for the day-to day operations of Care manateion including: Daily staffing scheduling; Supervising and mentoring staff; Assuring the standardization of care management; Assisting in responding to customer requests and problems; Maintaining staff performance informally through coaching, mentoring and providing developmental opportunities and formally through performance appraisal and disciplinary processes; Provides leadership/monitoring for compliance and performance and performance improvement.

Job Requirements:

Bachelor's Degree 
Equivalent experience accepted in lieu of degree
Related certification 
Registered Nurse ,Current Ohio licensure
3-4 years experience in a related field

Job Responsibilities:

Participates as a member of the interdisciplinary team to ensure that selected clinical quality criteria are met. This includes discussions/education on the units with physicians and nursing staff related to core measure requirements.
Achieves a target query rate of 12% rovide supervision to social work staff and ensure all rules and regulations are followed.
CDMP worksheets are informative but concise and are professional in appearance and quality. "win" worksheets clearly and accurately reflect the DRG shift. Track is kept up to date to facilitate monthly reporting. Assist / manage team member daily schedules and adjust schedules as needed to ensure adequate coverage.
Interdisciplinary team rounding on units and MDR rounds to ensure team member and patient needs are being met.  Including collaborating with the physicians and ensuring patients' needs are addressed timely. 
Availability for team members to address any issues and concerns related to workflow or patient discharge disposition.
