Job Overview: Evaluates patients, plans for and provides therapeutic interventions in a transdisciplinary/team approach. Responsible for the communication and coordination of treatment programs with other team members including the patient/family. Provides work direction to assistants, interns and other support personnel, participates in program developments, continuous improvement and clinical education; and provides in-services to staff, hospital personnel and the community. Will also be expected to be a member of a collaborative team which is patient focused, functional outcome based and patient/family centered.
Job Responsibilities: Provides comprehensive evaluation, therapeutic management, goal modification, patient/family education, and ongoing discharge planning for our patients. Also provides referral for appropriate professional programs, periodic re-assessment, and development of goal driven treatment plans. Provides all these services while taking into consideration the patient's age, educational level, family support and cultural and psychosocial issues. Maintains all required documentation within time frames and guidelines established by the department. Maintains established departmental productivity levels and manages time effectively. Contributes to department through involvement in continuous improvement programs and/ or special projects. This may include activities such as serving as a clinical instructor, providing professional educational content for peers, participation in team initiatives, or other areas approved by management. Remains educated on and in compliance with the laws and regulations that apply to the practice of Physical Therapy. Furthers personal knowledge base, clinical expertise, and professional development by attending continuing education offerings, independent reading, hands-on practice, individual training sessions, etc. Manages caseload efficiently and is able to prioritize competing requests for therapy. Able to collaborate with other therapists in order to satisfy service requests in a timely manner.
