This position assists physicians and/or registered nurses in delivering clinical care and supervising medical assistants, collaborating with healthcare professionals to address patient and family needs. The LPN implements care plans, providing direct nursing care in accordance with the Nurse Practice Act, contributing to patient assessments, and ensuring accurate, timely documentation. Location

Work Hours
- Monday, Tuesday, Wednesday, and Friday: 8:00 AM – 5:30 PM
- Saturday rotation: Every 7th Saturday, 8:00 AM – 11:00 AM

Qualifications
- Graduate of an approved nursing program (technical, professional, or vocational)
- Licensed Practical Nurse (LPN) or Current RN Licensure, Basic Life Support (BLS) for Healthcare Providers
- Ability to work collaboratively within a multidisciplinary team
- 1–2 years of experience in a related field
Key Responsibilities
- Medication Administration: Safely prepare and administer medications, ensuring compliance with legal and professional standards. Accurately interpret and execute physician orders, document timely, and educate patients and caregivers.
- Technical Proficiency: Perform clinical tasks aligned with hospital policies, competency standards, and best practices.
- Supply Management: Identify and fulfill departmental supply needs, ensuring seamless operations.
- Professional Development & Collaboration: Support nursing education, maintain current industry knowledge, and adapt to organizational updates.
- Record Coordination: Manage monthly turnover of Medication Administration Records (MARs), assisting in new participant MAR development.
- Patient Data Collection: Gather observations to contribute to nursing assessments, document findings, and communicate patient progress to the healthcare team.
- Regulatory Compliance: Demonstrate knowledge of PACE regulations related to primary care services for participants.
