Clinical Dietitian Specialist II - NICU

Job Overview:

Responsible for the provision of medical nutrition therapy for preterm and high risk newborns in the NICU and assist with nutrition assessment of patients followed in High Risk Clinic as needed. This includes assessment of growth, protein, electrolyte and mineral status, tolerance of enteral feedings and appropriate fortification of human milk and formula feedings. Participates in patient education through both one on one and group education opportunities provided by the NICU. Provides education for residents, fellows, neonatal nurse practitioner students, and staff. Participates and leads in process improvements in the unit and QI work within the Perinatal Institute. Maintains competency in pediatric and nutrition support practices, adhering to hospital and other accrediting policy. Oversight of infant feeding practices as it pertains to safe handling and preparation. Collaborates with the medical team and nursing to improve health outcomes of populations served within the NICU. Interfaces with nursing on all levels, working with NICU Unit Educator and Nursing Leadership and provides informal instructions to individual nurses. Collaborates with team members within the Perinatal institute to encourage and support advancements in nutritional care and outcomes of newborns and premature infants. Responsible to development of and revision of nutrition care and formula room policy and procedural guidelines, training and ensuring competency of NICU dietitians, milk techs. Assist in the training of PCA's who are responsible for assisting in infant feeding preparation.
Responsible for afterhours and weekend on call support for the medical team and unit.


Job Requirements:

As of 1/1/24, new practitioners are required to have a Master’s Degree in Dietetics.  If eligible or registered prior to 1/1/24, Bachelor’s Degree is Dietetics is required;

Certified specialist in Pediatrics or Certified Nutrition Support Clinician 
Dietitian 
CDR Registration & Ohio Licensure
Neonatal Dietetics and Nutrition Support 
Previous Pediatric experience
3-4 years experience Clinical Pediatric Dietetic

Job Responsibilities:

Provides nutritional assessment, intervention, and monitoring of all infants admitted to the NICU; including parenteral and enteral nutrition, assessment of growth and growth outcomes, laboratory monitoring, and patient education. Guides the transition of infants to enteral nutrition, including the development of specific formula alterations. Monitors at risk infants for physical and biochemical evidence of nutritional inadequacies. Collaborates with multidisciplinary team to ensure proper growth and nutritional interventions for preterm and high-risk newborns throughout the stay and upon discharge from the NICU. 
Maintains TPN program. In conjunction with pharmacy and medical director, addresses concerns related to changing practices or shortages of components of TPN. Educates residents and nurse practitioners on ordering TPN for preterm and high risk newborns. Educates nursing staff on fluids, TPN and enteral nutrition. Assists the RN's ability to spot errors when TPN is hung. Reviews all complex fluid and parenteral nutrition orders to ensure accuracy and nutritional completeness. 
Manages the functionality of the Breastmilk scanning program. Collaborates with NICU management, IS, and Timeless to address issues as they arise with the system. Develops and communicates changes to improve practices around scanning and documentation of infant feedings. Develops and maintains policies and procedural guidelines for the infant feeding preparation room ensuring education and competency of milk techs and PCA's responsible for the preparation of infant feedings. 
Manages and ensures training and competency of new dietitians hired to work in the NICU. Provides didactic education each month for new Pediatric resident teams to prepare them for caring for the nutritional needs of preterm and high risk newborns. Assists NNPs (Neonatal Nurse Practitioners) with nutrition education of NNP students and newly hired NNPs. As needed, provides nutritional consultation and support to the High Risk Clinic and Bethesda North Special Care Nursery physicians and NNPs.
Participates in QI work within the Perinatal Institute and for TriHealth. NEC (Necrotizing enter colitis) taskforce, maintains data base and data collection for infants on the feeding protocol and cases of NEC. Reports monthly to the NEC taskforce team. Participates and assists in QI work within OPQC as it relates to nutritional practices.
As needs arise, assists in research studies related to nutritional studies in preterm infants, human milk, or growth. Assist the researcher(s) in understanding feeding preparation and practices that allow them to gather appropriate data in a systematic way that yields best information.
