HEALTH COACH II: This position is responsible for providing face-to-face, telephonic and/or internet-based coaching services to clients with complex, chronic, or multiple co-morbid medical conditions across the wellness-illness continuum. The population focus is on individuals who have been diagnosed with chronic conditions and have multiple barriers impeding their ability to engage in healthy lifestyle behaviors. The Health Coach II will have a minimum essential skill set of an ability to assess/ascertain client needs, establishment of rapport with a varied clientele, motivational interviewing skills, SMART goal definition and application, identification of measurable behavior change and clear understanding of community/support resources available to clients. Additionally, this position will have a basic knowledge foundation of common medical chronic conditions, such as diabetes, hypertension, obesity, depression, etc. The coach must be able to effectively assess a client's readiness for change and address multiple goals within a framework that addresses complex barriers to behavior change. Recognition and understanding of individual strengths and limitations, emotional intelligence and provision of a nonjudgemental environment is essential. The coach must demonstrate success in individualizing the coaching process in order to move this population towards sustainable lifestyle behavior changes including self-empowerment and development of a stable social support system. Achievement of this type of success encompasses successful navigation to a myriad of resources, such as mental health counselors, pharmacists, dietitians, clinical educators, etc. Additionally, the Health Coach II analyzes and understands the individuals use of pharmaceutical modalities, medication adherence, inpatient and emergency room utilization and can integrate lower cost options within the SMART goal process to positively effect the individual's medical spend trend from high cost utilization to lower cost options. MINIMUM REQUIREMENTS: EDUCATION: Bachelor's Degree Other Nutrition, Behavioral Healthcare, Health Education, Exercise Science or related field Specialty certification/licensure in health/wellness area.Other EXPERIENCE: 2-3 years Customer Service Other Coaching experience in a health related field (i.e. health, wellness, behavioral counseling) SPECIALIZED KNOWLEDGE: Principles of health behavior change, coaching, wellness; ability to maintain goal orientation despite barriers to attainment; understanding of extrinsic/intrinsic motivation, readiness to change, ability to measure behavior change.
