BEREAVEMENT COUNSELOR: This position is responsible for providing individual and group counseling for HOC bereaved family members/caregivers as well as bereaved clients from the community coping with the death of someone close. This position is responsible for contacting all of the bereaved who have been designated as "high risk" by the HOC direct patient care team members. As part of the contact with the "high risk" clients, a bereavement assessment will be completed including identification of current risk factors, such as limited resources; limited emotional support; children in the home; multiple losses and/or history of compromised coping. The counselor provides short term grief counseling based upon bereaved's grief issues and concerns. Counselors faciltitate a variety of grief support groups which vary in length of time as well as issue complexity. This position will also work closely with the direct patient care team when providing counseling support to active HOC patients/family members to ensure a smooth transition to the bereavement counselor upon the HOC patient's death. This position will also refer to community professionals when there are issues related to life long patterns of behavior or concerns.



EDUCATION: Master's Degree Counseling and/or Social Work, Licensed Social Worker

EXPERIENCE: 4+ years Professional Counseling and/or Social Work

SPECIALIZED KNOWLEDGE: Knowledge of counseling principles with children and adults. Knowledge of community resources and training in grief and loss, crisis intervention, stress reduction. Computer aptitude and knowledge of computerized medical records.

DEPARTMENT SPECIFIC QUALIFICATIONS: This position works for Hospice of Cincinnati. The office is located in Blue Ash. Licensed Independent Social Worker and/or Licensed Practical Clinical Counselor - Licensed in the State of Ohio.  Prior Counseling Experience - Grief Work Preferred.  Ability to provide services across the lifespan - Children, Adults and Seniors.
